In-Network Insurance
I am an in-network provider for Premera, Regence, and Lifewise.
Verify Your Benefits
If you have one of these plans, please contact your insurance company directly to confirm your specific reimbursement rates for mental health care.
Seamless Billing
For your convenience, I will bill your insurance provider directly at the end of each month.
Co-pays
You are responsible for your co-pay. Once your insurance has reimbursed the service, I will bill you for any remaining co-pay balance.
Out-of-Network Care
If your insurance is not Premera, Regence, or Lifewise, I will be considered an out-of-network provider.
Session Fee
The fee for out-of-network services is $175 per session (50–52 minutes), payable at the end of each appointment.
Reimbursement (Superbills)
I will provide you with a Superbill (a detailed medical receipt) that you can submit to your insurance company for potential reimbursement.
Your Responsibility
Please contact your insurance provider directly to confirm their policies and reimbursement rates for out-of-network mental health care.

- すべてのセッションは、完全予約制となっております。
- お支払いは、クレジットカード決済のみにて承ります。
保険の適用について
当カウンセリングは、対話を通じた自己成長と心の回復を目的とした専門的なサポートです。そのため、医療保険の適用はございません。また、健康保険等の請求に必要な「スーパービル（診察明細書）」や診断書の発行も対応いたしかねますので、あらかじめご了承ください。
キャンセルポリシー
予約の変更やキャンセルは、セッション予定時刻の [例：48時間] 前までにお知らせください。それ以降のキャンセル、または無断キャンセルの場合は、誠に恐縮ですが以下の通りキャンセル料を申し受けます。
• [48時間] 前〜前日までのご連絡： セッション料金の [50]%
• 当日のご連絡・無断キャンセル： セッション料金の [100]%
※ ただし、急な体調不良や自然災害など、やむを得ない事情がある場合はその限りではありません。どうぞお早めにご相談ください。
予約の流れ
- 初回相談のご予約： ページ下部のフォームよりお申し込みいただくか、直接メール（tamamif@tamamifujiwara.com）でお問い合わせください。
- 事前書類のご確認： ご予約確定後、まずは同意書などの書類をお送りします。初回セッションの2日前までにご確認、ご記入、ご署名をいただき、ご返送いただければと思います。
- セッション当日： 事前にオンラインのリンクをメールで送信いたします。当日は、指定のリンクよりお入りください。
